Expanding Permanent Pathways In Medicare For Accountable Care

MCP is testing changes to primary care financing by helping practices move from fee-for-service to hybrid and population-based payments, ways to advances equity, with 41 percent of participants being federally qualified health centers, and seeks to support sustainability through alignment with Medicaid and commercial payers in participating states.
